We support eligible individuals who may benefit from structured residential services, person-centered skill development, companionship, and authorized home or community-based assistance. Our team works with individuals, families, guardians, support coordinators, and referral professionals to determine whether our services are an appropriate match.

Serving specific populations does not mean every service is available to every individual. The exact services offered depend on authorization, assessment, and our approved provider scope.
Eligibility
How eligibility is determined
Program requirements
Each service line has its own program requirements. Residential Habilitation, for example, follows specific eligibility rules that differ from home & community support.
Role of APD or other authorizing programs
For applicable services, authorization from the Agency for Persons with Disabilities (APD) or another authorizing program may be required. [Our current APD provider status and any other program participation to be confirmed.]
Service-plan requirements
Services must align with the individual's approved support plan. Our team collaborates with support coordinators to ensure this alignment.
